Very nice! I was working one one that used a FAL type hold open by the trigger, i planned to manipulate it the same way as with the fal.
I really like this, and it looks like it belongs there!

What about a misfire or failure to fire? Do you have to manipulate it every time that you pull the charging handle?

No I installed a small spring around the pin that keeps the charging handle attached (from the FA trigger group I believe) that keeps the front of the lever snug on the receiver. So by default it hooks the front of the ratchet plate and keeps the CH up front until I need it. You have to manually engage the BHO.
